OUR TEAM

The people behind your care.

When you receive care through health2me, you get all of us. One team across all three programs, so whoever you see, in the office or at home, already knows your name and your story. You're part of this team as much as we are. Here are the people you'll be working alongside.

Your guide between visits

Kate Miyashita, CNA

Care coordinator

Your doctor is always your doctor, and always within reach. But between visits, Kate is the one who keeps everything moving. She has spent her whole career in long-term care, including years at Kaiser on the mainland, so she knows how the system works and how to move through it on your behalf. She is also a gifted caregiver: as steady and compassionate as she is knowledgeable. Whether you need someone to untangle a confusing benefit or a real answer to an everyday care question at home, Kate makes sure you and your family never have to figure it out alone.

human-centered innovation in healthcare

"I started health2me because I noticed healthcare was built around the efficiency of the system, and it wasn't built around the patient. Taking a human centered design approach, we designed health2me to deliver care where people need it, when they need it, and in a way that actually makes a difference in their health."

Dr. Christopher Wong, founder and managing director of health2me Clinic Care
