This Privacy Policy (“Policy”) describes how health2me (“Company,” “we,” “us,” or “our”) collects, uses, discloses, and safeguards personal information, including Protected Health Information (“PHI”) as defined under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”), in the course of operating our business, including through our website located at www.health2me.com (the “Site”) and in connection with our in-home medical services.

1. Information We Collect
We may collect personal information such as your name, email address, telephone number, payment information, and any other information you voluntarily provide to us through forms, communications, and service requests. In addition, as a provider of in-home medical services, we may collect PHI, which includes information relating to your past, present, or future physical or mental health condition, the provision of healthcare to you, and payment for such healthcare services.
2. Methods of Collection
We collect personal information and PHI directly from you when you submit information through online forms on our Site, subscribe to newsletters or other communications, request services, or make a purchase or payment. We may also collect such information through secure communications with you during the course of providing healthcare services.
3. Purpose of Collection and Use of Information
We use personal information and PHI for the purpose of providing and administering our in-home medical services, including home-based primary care and private duty nursing services. Such uses include responding to inquiries, providing customer support, processing payments, coordinating and delivering medical care, and sending marketing and promotional materials where permitted by law. With respect to PHI, we use such information solely as permitted under HIPAA for treatment, payment, and healthcare operations, or as otherwise authorized by you in writing.

We do not sell personal information or PHI. We may disclose personal information to third parties who perform functions on our behalf, such as payment processors and service providers necessary to operate our Site and deliver our services. PHI will be disclosed only as permitted or required under HIPAA, such as to healthcare providers involved in your treatment, to entities involved in billing and payment processing, or to government authorities as required by law.
5. HIPAA Compliance and Patient Rights
As a covered entity under HIPAA, we are required to maintain the privacy and security of your PHI and to provide you with a Notice of Privacy Practices, which describes in greater detail your rights with respect to your PHI. These rights include the right to access and obtain a copy of your PHI, request an amendment to your PHI, request restrictions on certain uses and disclosures, and receive an accounting of disclosures. You also have the right to request communications of PHI by alternative means or at alternative locations, and to file a complaint if you believe your privacy rights have been violated. We will not retaliate against you for filing a complaint.

We retain personal information and PHI for as long as necessary to fulfill the purposes for which it was collected, to comply with applicable legal and regulatory requirements, to resolve disputes, and to enforce our agreements.
7. Data Security
We implement administrative, technical, and physical safeguards that comply with HIPAA and applicable state privacy laws in order to protect personal information and PHI against unauthorized access, use, alteration, or disclosure. While we take these precautions seriously, no security measure can guarantee absolute protection.
