Lauren Azuma, MD
Family Medicine Physician
Dr. Lauren Azuma is a board-certified family medicine physician who believes the best care begins with trust and a genuine connection. Born and raised in Hawai‘i, she studied at the John A. Burns School of Medicine and completed her residency in Family Medicine at John Muir Health in the San Francisco Bay Area.

Known for her calm, thoughtful approach, Dr. Azuma takes time to truly understand her patients, their lives, their homes, and their goals. She also teaches advanced clinical skills at the University of Hawai‘i, continuing her commitment to compassionate, community-based care.

Kanagusuku, MD
Family Medicine Physician
Dr. Leimomi Kanagusuku is a family medicine physician with deep roots in Hawai‘i. She grew up on the West side of O‘ahu, graduated from the John A. Burns School of Medicine as class president, and completed her Family Medicine residency at the University of Hawai‘i.

Dr. Kanagusuku is passionate about building lasting relationships with her patients and delivering care that respects the whole person. In addition to seeing patients, she helps train the next generation of physicians as Associate Program Director for the UH Family Medicine Residency and serves her community as President of the Hawai'i American Family Physicians.

Kate Miyashita, CNA
Care Coordinator
Kate comes from a long line of caregivers and grew up in a family full of care home operators. She has extensive experience helping to coordinate care for her residents and patients in the community, both here in Hawaii and on the mainland. Not only that, she brings compassion to her work and truly connects with the people she works with. Kate is here to help guide you through your medical journey.

Christopher Wong, MD
Office Manager
Chris brings a blend of engineering and empathy to healthcare. He studied Biomedical Engineering at Columbia University and co-founded Medical Innovation and Design (MIND) Hawaii, where he focused on making care more intuitive and patient-centered. After earning his medical degree from the John A. Burns School of Medicine, Chris turned his attention to rethinking how we engage with healthcare — designing healthcare experiences that feel less like factories, and more like care.
